1. Foreman is notified immediately
2. Injury life-threatening? Dial 911
3. If not life threatening, Foreman to call Medcor at 1-800-775-5866 to determine next steps
4. Foreman to notify Kevin Nelson 602-339-3537
5. Foreman to notify Christine Kettelkamp 480-772-8437
6. Foreman to notify GC
7. Foreman to complete the Incident Report Form before the end of shift
8. Conduct Incident Investigation with PRM
